This is the registration form for the sacrament of Confirmation. 

 

Confirmation Classes will start on May 4 (Sun), at 12:30-2:00 PM

 

Please prepare as well, a copy of the candidates baptismal certificate. 

 

The date for Confirmation is on June 8 Pentecost

 

After you fill up the form you can send it through email or bring the registration form on Sunday.
